Nurse practitioner provides services through the performance of patient evaluation, assessment and treatment in the medical office or home setting as allowed by state licensure laws and under the supervsion of the administrator with input from the medical director.

  • Graduate of recognized program for Nurse Practitioner training (MSN).
  • Able to communicate effectively with medical and hospice staff and various populations within Tanner Health System and in the community.

Functions

Area of Responsibilities

  • Performs face to face encounter evaluations of hospice patients to determine eligibility of hospice benefits.
  • Provides findings of face to face encounters to the medical director prior to the re-certification date.
  • Completes face to face narrative for re-certification and any other hospice required documentation prior to the re-certification date.
  • Maintain evidence based medical care and clinical charting. Participates with clinical tracking and feedback.
  • Reports findings of the face to face visit within 24 hours to the hospice administrator or designee.
  • Maintains and updates level of skills as pertinent to the nurse practitioner licensure requirements.

Compliance Statement

Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.

  • Must maintain high degree of confidentiality.
  • Must be able to deal tactfully and effectively with patients, family members, other employees and physicians.

Position Responsibilities

Effect of Error: Probable errors not easily detected and may adversely affect external as well as internal relationships and may result in major expenditures for equipment, materials, or procedures detrimental to the patient's welfare or the organization's interest. Work is subject to general review only and requires considerable accuracy and responsibility. Continually works with reports, records, plans, and programs of a major functional area of the organization where integrity is required to safeguard the organization's position. Duties may involve the preparation of data on which the administration bases important decisions and are highly confidential.

Working Conditions: Moderate - (About 50% of the day) Involved in exposure to dirt, odors, noise, or some work is performed with exposure to temperature/weather extremes/occupational risk. Limited probability of coming into contact with blood borne pathogens, other potentially infectious diseases, or biomedical/bio-hazardous materials.
