Qualifications:
Area of Responsibilities
* The Data Abstractor will perform data collection for the TMG Patient Centered Medical Home, Patient Centered Specialty Practice and Patient Center Connected Care. This information must be accurately compiled. This data will be collected from Epic electronic medical records and trended in spreadsheet formats to assist with the outcome measurement.
* Facilitation: The Data Abstractor works with the Patient Outreach Representatives to close patient care gaps by utilizing payor portals to track progress.
* The Data Abstractor will assist rural health clinics in their annual program's evaluation chart reviews as well as immunization reimbursement reporting.
* The Data Abstractor will assist with Press Ganey/NRC reports for leadership in order to improve patient satisfaction.
* Communication: The Data Abstractor will communicate effectively with leadership, internal and external clients. The Data Abstractor will have on-going communication with staff, medical staff, and leadership.
* Monitors program compliance for quality programs such as Patient Centered Medical Home and Rural Health requirements.
* Position may involve patient contact in an effort to coordinate needed care and close care gaps.
* Assist with the development of policies and procedures to ensure compliance with quality and regulatory standards.
* Serves as resource for various projects related to cost reports and other quality initiatives.
* Participates in Patient Centered Medical Home workgroups to prepare for certification and renewals. Works with leadership to prepare materials for workgroup meetings.
* Collaborates with management team and staff to identify and implement corrective action when needed.
* The Data Abstractor is a subject matter expert on requirements for Patient Centered Medical Home and other quality initiatives.
Compliance Statement
Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.

Definitions
*The Data Abstractor works with the leadership team to monitor program compliance with Patient Centered Medical Home standards. The Data Abstractor compiles data, assists with the development of policies and procedures, and coordinates data collection for other regulatory requirements and projects.
