What are the responsibilities and job description for the Clinical Care Coordinator - Transfer and Bed Control (Nights) position at Tanner Health?
The Hub Clinical Care Coordinator for the Tanner Transfer Bed Management Center will use acquired knowledge and experience to facilitate transfers and acceptance of patients for all facilities within Tanner Health System. The position collects clinical information relative to a transfer request and then facilitates the transfer and bed request by matching the patient's clinical needs to the facility with bed capacity and service capability. This position works in collaboration with multiple departments within each hospital to foster relationships with referring facilities, physicians, and hospital staff.
Required Knowledge & Skills
Education: Associate Degree or 2 years of college coursework
Experience: Three years of related experience. Requires working knowledge of specialized practices, equipment, and procedures.
Licenses and Certifications
GEORGIA REGISTERED NURSE LICENSE OR MULTISTATE NURSING LICENSE

Georgia or multistate RN licensure
