Job Description

Responsible for assessment of all psychosocial factors and participation in treatment planning of patients/families.

Required Knowledge & Skills

Education: Master's Degree

Experience: One year of related experience. Requires a working knowledge of standard practices and procedures.

* MASTERS SOCIAL WORKER

Qualifications

* Masters Degree in Social Work required.

* Experience - minimum of one year in hospital or medical setting.

* Ability to deal comfortably with people in emotional need in a competent, calm manner.

* Proficiency in communication skills at all levels.

* Proficiency in observation, assessment and problem-solving skills.

* Demonstrates flexibility, initiative and organization in performance.

* Ability to work independently.

* Recognition and commitment to continuing profession development (i.e., professional affiliations: NASW, ACSW, GSHSW, licensure).

* Flexible schedule, some evening work may be required. Pager rotation for evening and weekend coverage required.

Functions

Area of Responsibilities

* Assesses psychosocial needs and completes assessment as indicated including recommendations for treatment plan.

* Participates in team planning regarding psychosocial adjustment and continuity of care needs.

* Educates team members in understanding significant social and emotional factors related to care of terminally ill.

* Evaluates coping skills and provides crisis intervention as needed.

* Provides individual or group counseling services to patients/families/community as indicated.

* Maintains knowledge of and liaison with community resources for information and referral.

* Assesses financial situation and provides appropriate resources as indicated.

* Prepares timely documentation for medical record and maintains current case notes activity statistics.

* Actively participates in development of department quality improvement activities.

* Works closely with Volunteer Coordinator to participate in the orientation, training and coordination of Hospice volunteers as appropriate.

* Organizes and participates in community education concerning grief, bereavement and psychological needs of the terminally ill patient and their family.

* Provides bereavement support to families.

* Maintains Social Work policy of confidentiality and adheres to NASW ethical standards.

* Participates in continuing education as mandated per policy.

* Participates in/conducts departmental projects.

* Demonstrates competency in patient care according to the age of the patient and population served.

* Complies with standards set forth by TJC and other regulatory agencies.

Compliance Statement

Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.

Position Responsibilities

Contact with Others: Requires frequent contact with many persons at different levels inside and outside of the organization to carry out organization policies and programs and obtain willing acceptance, consent, or action.

Effect of Error: Probable errors not easily detected and may adversely affect external as well as internal relationships and may result in major expenditures for equipment, materials, or procedures detrimental to the patient's welfare or the organization's interest. Work is subject to general review only and requires considerable accuracy and responsibility. Continually works with reports, records, plans, and programs of a major functional area of the organization where integrity is required to safeguard the organization's position. Duties may involve the preparation of data on which the administration bases important decisions and are highly confidential.
