Job Description

A key member of the pharmacy department team, this position requires a wide array of interpersonal as well as organizational skills. This person is responsible for accurate management of dispensing and clinical functions within the department, supervising technician and support personnel, and provision of appropriate patient care as a member of the pharmaceutical care team. Job responsibilities and assignments may vary with demonstrated skills and qualifications.

Requirements

Area of Responsibilities
*Completing pharmacokinetics and other medication dosing consults accurately, in a timely manner, and providing follow-up per THS protocol. Completing TPN consults accurately, in a timely manner, and providing follow-up per THS protocol.
*Ensuring safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols. Monitoring of drug therapy regimens for contraindications, drug-drug and drug-food interactions, allergies, and appropriateness of drug and dose. Reading, extracting, and interpretation of information from patient charts.
*Interpretation of medication orders (verbal and written) and transcription to computerized patient medication profiles accurately and in a timely manner. Maintenance of accurate, complete patient medication profiles.
*Preparation and dispensing of medication orders per physician request according to established policies, procedures, and protocols in an accurate and timely manner.
*Verification of the daily activities and medication-related duties of the pharmacy technician and support personnel staff.
*Ability to adapt positively to frequent interruptions and changes in workload and/or work schedule.
*Ability to organize and prioritize work assignments.
*Ability to work independently with minimal supervision.
*Appropriate and timely answering of the telephone and requests at the pharmacy window.
*Attendance at Pharmacy Department staff meetings.
*Clinical consultation provision to healthcare practitioners, accurately and in a timely manner.
*Compounding and dispensing of pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition products accurately.
*Demonstration of sound professional judgment consistent with clinical/academic background.
*Detection and reporting of suspected adverse drug events.
*Documentation of clinical activities and interventions.
*Formulary maintenance. Minimization of non-formulary procurements, utilization of therapeutic substitution protocols, and promotion of rational drug therapy selection.
*Issuing controlled substances to patient care areas. Maintenance of accurate controlled substance records, as required by law.
*Participation in the development and presentation of orientation, education, and training programs to the Pharmacy Department, medical, nursing, and other staff, as assigned.
*Participation in the quality improvement and medication use review activities of the department.
*Provision of drug information to patients and families, accurately and in a timely manner.
*Review and interpretation of culture and susceptibility data for antibiotic appropriateness. Recommendation of changes to therapy, as needed.

Compliance Statement

Probable errors may be serious and involve losses such as improper costs, overpayment, waste of material, damage to equipment, and delay in processing work. Effect usually confined within the organization. Most of work not subject to direct verification or check. Regularly works with some confidential data such as account, salaries, patient medical records, which if disclosed might have adverse internal or external effects.

Supervisory Responsibility
Has limited supervision over a small section of employees (up to 15), i.e. assigns and directs their work; instructs new employees and corrects results, may discuss with supervisor, but has no authority over job content or personnel. Team governance where the Members evaluate, interview, and recommend personnel actions.

Work involves a variety of problems in a general field, some of which are complex. Involves some independent judgment to decide what to do to assemble facts, determine variations from standard procedures, or plan other action to be taken to meet general objectives.

Minor physical effort - Job requires person to stand and/or walk frequently. Lifts, carries, or uses lightweight (1 to 25 lbs.) materials or equipment less than half of the day. Works in reaching or strained position intermittently. Office or laboratory work requires close visual effort less than half of day. Office or Laboratory work with concentration on a monotonous, repetitious procedure or skill most of day, where speed and accuracy are essential.

Minor - Occasionally involved in exposure to dirt, odors, noise, or some work is performed with exposure to temperature/weather extremes/occupational risk and probability of coming into contact with blood borne pathogens, other potentially infectious diseases, or biomedical/bio-hazardous materials.
