A health services network in New York City is currently seeking a licensed Registered Nurse (RN) to work with the IDD population at one of their group homes in Brooklyn.About the Opportunity:Schedule: Monday to FridayHours: 9am to 5pm (1/2 hr unpaid lunch)Setting: Group HomePatient Population: Adults (IDD)Responsibilities:Coordinate the delivery of routine and specialty health care services to address assessed needs of persons receiving services in assigned program unitAssess and establish health care outcomes for persons receiving services; update assessments on at least a semi annual basis or as indicatedAct as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providersComplete all required written documentation; chart healthcare progress as indicated by acute illnesses and/or complex ongoing medical problemsEvaluate the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessaryConduct general and individual specific health related trainings for persons receiving services, direct care staff, and other team members as neededPerform other duties, as neededQualifications:1 years of Nursing experienceGraduate of an accredited School of NursingNYS Registered Nurse (RN) licenseSolid assessment, clinical, and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills:Bachelors Degree in NursingExperience working with persons with Developmental Disabilities
