Current Openings:
Full-Time: Monday through Friday (8:00 AM to 4:30 PM)
The Social Worker is responsible for delivering high-quality social work services to individual residents, taking into account their unique needs, preferences, abilities, challenges, and family circumstances. This position will be performed in accordance with TANDEM LIVING's Core Values, Mission Statement and regulations.
QUALIFICATIONS:
A B.S.W., B.S., or B.A. in Social Work or Behavioral Sciences is required.
At least one (1) year of experience working with the geriatric population is preferred.
- Basic computer skills required, including proficiency in Microsoft Office (Word and Outlook).
RESPONSIBILITIES INCLUDE:
- Serves as a senior member of the care team, focusing on residents’ psychosocial needs and behavioral concerns. Completes Person-Centered (PC) assessments as required.
- Supports new resident onboarding, assisting with adjustment to the facility. Assigns a resident ambassador and meets with new residents to provide orientation and a facility tour.
Addresses factors impacting residents’ quality of life, including roommate concerns, understanding changes in level of care, coping with illness or the aging process, grief, and other personal or emotional challenges.
- Maintains ongoing communication with residents’ families by following up after admission, providing facility orientation, and serving as the primary contact for psychosocial needs such as roommate concerns, changes in level of care, illness, adjustment to the aging process, grief, and other related issues.
