Duties And Responsibilities

  • Develops comprehensive, individualized care plans for pediatric patients and their families.
  • Provides ongoing follow-up and support to assist families with maintaining engagement in care, attending scheduled appointments, and accessing recommended services.
  • Builds rapport and trust with children and caregivers and engages families in collaborative problem-solving.
  • Performs trauma-informed psychosocial assessments.
  • Partners with Enabling Services to provide families with access to community resources and applying for state and federal programs including, but not limited to, Medicaid, SNAP, disability services, early childhood programs, and school-based supports.
  • Integrates social and medical services in a holistic, family-centered approach to pediatric care.
  • Maintains accurate and timely documentation of services, referrals, bills appropriately for services rendered, and other required patient-related information in the electronic medical record and program data systems.
  • Participates in learning collaborative activities, including team meetings, quality improvement projects, training sessions, and data reporting.
  • Adheres to organizational policies, social work standards, confidentiality requirements, and trauma-informed care principles in all work activities.
  • Performs other duties as assigned by supervisor.

Qualifications

Education

  • A master's degree from an accredited college or university in Social Work or related field (MSW); AND
  • Possesses an active, unrestricted license in one of the following disciplines: Licensed Master Social Worker (LMSW), Licensed Professional Counselor (LPC), or Licensed Independent Social Worker (LISW-CP).

Knowledge, Skills And Abilities

  • Knowledge of Adverse Childhood Experiences (ACEs), trauma-informed care, child development, and family systems.
  • Knowledge of major social services, community resources, and child- and family-support programs.
  • Prefer knowledge of/experience with social institutions, including policies and procedures related to Medicaid, SNAP, disability services, and educational support programs.
  • Ability to work independently with minimal supervision while managing multiple priorities.
  • Ability to work effectively in a team-based environment.
  • Ability to communicate clearly, engage families compassionately, and collaborate with clinical staff.
  • Ability to follow procedures, maintain accurate records, and utilize electronic medical record systems.
  • Ability to adjust to evolving program needs and collaborative activities.
  • CPR certification is preferred.

Environment

Very busy pediatric clinical atmosphere. Potentially stressful when working with children, families, and their unique needs. Rare potential exposure to blood-borne diseases (Category II). Rare exposure to chemical hazards (Category B).

Physical Demands

Considerable sitting, standing, or walking. May assist with movement of supplies or child-related equipment as necessary. Lifts supplies/equipment up to 20 lbs occasionally, and/or up to 10 lbs frequently. Considerable reaching, stooping, bending, kneeling, and crouching. Interaction at child-appropriate heights is frequently required.
