Job Summary: The Social Services Coordinator II collaborates with primary care providers to assist patients with accessing social services available within TCC and in the community. Coordinates referrals to resources and social services for patients based on an assessment of barriers to meeting the goals of their medical plan of care. Assists patients with social problem solving in order to create stability in their lives, so they may achieve their desired health status. Works in a multi-disciplinary team-based system of care providing services for all beneficiaries of TCC Health Services. Acts as a liaison between patients, family & caregivers, social service agencies, and the patients’ primary care providers. Applies knowledge and experience in social services to further the achievement of TCC’s vision for an Alaskan Native community that enjoys physical, mental, emotional, and spiritual wellness, and the fulfillment of TCC’s mission of ensuring healthy people across generations.

Essential Functions

Representative Duties: Under the direct supervision of the Social Services Manager the job incumbent will:
1. Provides social services coordination based on referrals from medical providers and other sources. Responds by connecting patients with available resources to meet the identified need both in Fairbanks and throughout the TCC region.
2. Identifies, tracks, and follows up with patients who need social support in order to engage in their health care and achieve the goals of their medical plan of care. Maintains awareness of the current status of referrals of care in order to direct and assist the primary care team.
3. Assists the patient with navigating the available systems of services to meet their identified needs. Provides direct support for patients who need help completing applications for assistance and enrolling in available social service programs.
4. In conjunction with other team members, and by addressing any social barriers, assists patients with appointment scheduling, medical records transmission, travel coordination, verification of PRC payment arrangements, and follows up on referral results.
5. Ensures closed-loop communication with clinical staff and referring providers so that all members of the patient’s care team are informed and aware of the services the patient is receiving to address identified barriers to care.
6. Communicate by phone, electronically, and in person with patients, medical providers, nurses, and other support staff to coordinate services. Manages the receipt and transmission of protected health information (PHI) as needed to facilitate care.
7. May be required to temporarily fulfill the duties and supervisory responsibility of the Social Services Manager on an as-needed basis.
8. Participates in program planning and quality improvement projects related to the quality of care and support provided to TCC patients.
9. Willingly accepts and performs other job-related duties as assigned or required.
10. May be required to travel to communities within the TCC region to provide coordination of services directly to patients.

Minimum Qualifications
Minimum Qualifications:
1. GED or high school diploma required.
2. Two years of work experience providing direct social services coordination to clients in a social service or health care organization.
3. Ability to maintain patient confidentiality, interact effectively with patients and fellow staff and communicate clearly. Excellent verbal and written communication skills.
4. Strong critical thinking and decision-making ability.
5. Must be dependable, prompt, motivated, and able to work under minimum supervision.
6. Strong affinity for team-based work and collaborative problem-solving.
7. Must pass background check under the State of Alaska background check and federal Indian Child Protection and Family Violence Prevention Act requirements.
Knowledge, Skills and Abilities:
1. Knowledge and understanding of co-occurring related issues.
2. Familiarity with Alaska Native Traditions and Cultures.
3. Familiarity with medical terminology and medical procedures.
4. Proficiency with PC-based office productivity software (Microsoft Office, Outlook, etc.) and ability to operate standard office equipment.
5. Functional awareness of cross cultural issues in Counseling.
6. Strong verbal communication, analytical and interpersonal skills required.

Supervision
Physical demands: The position is mostly sedentary with extended periods sitting at a desk/computer. Job incumbent must be able to lift and carry up to 50 pounds. Incumbent must be able to work efficiently in a busy and sometimes stressful environment. Shared office space in a clinical setting.
Work environment: Work is performed in a clinic responsible for treating patients with a wide variety of medical needs. The employee may be exposed to communicable diseases during the performance of their regular duties.
