Job Summary:** The job incumbent is responsible for the management and oversight of the Diabetes and WIC Program. This position provides leadership, vision, program management, and evaluation of program goals, activities, and staff. This includes completing all required grant activities and reporting for multiple federal grants and applying for new funding sources when appropriate. Job incumbent is also responsible for maintaining American Diabetes Association (ADA) education recognition accreditation, CDC Diabetes Prevention Recognition Program (DPRP) and Medicare approved DPP status (if applicable). Job incumbent works to promote wellness and prevention of chronic disease in all TCC communities by supporting the provision of direct diabetes education and nutrition counseling services both at CAIHC in Fairbanks and in rural communities throughout the region.

Representative Duties:** Under general supervision of the Director of Nursing job incumbent will:
Provides leadership and oversight of the Diabetes and WIC Program. Ensures alignment of program goals and activities in accordance with TCCs strategic plan and grant requirements including the Special Diabetes Program for Indians (SDPI), Women, Infant and Children (WIC), and other grants (as applicable). Ensures the delivery of diabetes and nutritional care and education in communities throughout the region.
Supervises the Diabetes and WIC Program staff according to the current TCC personnel policy manual and CAIHC employee manual. Ensure the program is staffed appropriately.
Oversees the planning, implementation, and evaluation of the diabetes self-management program (DSMES) including maintaining recognition status with the American Diabetes Association, CDC Diabetes Prevention Program (DPP) and Medicare DPP (if applicable).
Chairs the Diabetes Advisory Committee and facilitates coordination and integration of Diabetes Program services into clinical practices throughout the region. Acts as the subject matter expert in the field of diabetes by having extensive knowledge of the most current diabetes standards of medical care, treatment guidelines, and diabetes self-management education and support.
Advocates for the integration of chronic disease prevention and health promotion initiatives in the planning and development of activities and programs within TCC and in collaboration with other internal and external programs. Utilizes population health strategies to optimize patient-care and education.
Collaborates with other service providers to plan, coordinate, and participate in educational activities including community outreach events. Serves as a liaison with local, state, regional, tribal, and federal/national agencies and organizations.
Facilitates, directs, and contributes to quality improvement projects based on quality measures, identified areas of improvement, process/program evaluation, patient satisfaction, etc.
Develops and manages TCC Diabetes and WIC Program budgets within the requirements of funding sources and TCC Accounting guidelines.
Ensures completion and submission of all grant reporting requirements including SDPI, WIC, etc. to satisfy grant requirements and maintain program funding.
Applies for additional funding opportunities whe applicable to support and advance Diabetes and WIC Program services.
Travels to villages for providing community outreach and education.
Provides clinical coverage for diabetes self-management education when SDPI program clinical staff is out of the office.
Composes, initiates, and reviews current policies for the SDPI program team.
Maintains current professional licenses and certifications as designated by professional licensing board.
