Responsibilities:
- Evaluate patients in a timely manner following physician referral
- Develop effective treatment plans and provide education for clients, families, and caregivers as a subject matter expert
- Communicate with supervisors and other healthcare team members regarding patient progress, problems, and plans
- Participate in inservice training programs for other staff in the facility
- Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy
- Provide accurate and timely billing records and supporting clinical documentation
- Instruct patients' families or nursing staff in maintenance programs
- Coordinate necessary durable medical equipment and adaptive equipment for patients to facilitate safety and independence in ADL's
- Ensure resident abuse or neglect does not occur
- Observe confidentiality and safeguards for all patient information
- Comply with federal, state, and local legal and professional requirements
