This role involves developing personalized care plans, conducting assessments, and implementing therapeutic interventions to enhance patients' quality of life.

Duties
- Conduct comprehensive evaluations to assess patients' physical, emotional, and social needs.
- Develop and implement individualized care plans tailored to each patient's goals and capabilities.
- Provide direct patient care through therapeutic activities designed to improve daily living skills and overall functionality.
- Collaborate with other healthcare professionals, including speech therapists and case managers, to ensure a holistic approach to patient care.
- Educate patients and their families about treatment options, progress, and strategies for ongoing support.
- Monitor patient progress and adjust treatment plans as necessary based on observed outcomes.
- Maintain accurate documentation of patient assessments, treatment plans, and progress notes in compliance with policies.
Requirements
- Degree in Occupational Therapy from an accredited program.
- Valid state licensure as an Occupational Therapist.
- Strong knowledge of physiology and occupational health principles.
- Experience in case management and working with individuals with disabilities is preferred.
- Familiarity with early intervention strategies is a plus.
- Excellent communication skills to effectively interact with patients, families, and interdisciplinary teams.
- Ability to develop effective care plans that address individual patient needs.
- Compassionate nature with a strong commitment to patient care and rehabilitation outcomes.
