- Evaluate residents’ functional abilities and limitations
- Develop individualized occupational therapy treatment plans
- Help residents improve activities of daily living
- Address strength, coordination, balance and functional mobility
- Recommend adaptive equipment and environmental modifications
- Educate residents, families and caregivers
- Complete timely evaluations, progress notes and discharge documentation
- Collaborate with nursing, rehabilitation and interdisciplinary teams
- Participate in care planning and discharge preparation
- Holds an active Idaho Occupational Therapist license or has a pending license
- Has recent LTC, SNF or rehabilitation experience
- Can manage treatment planning and clinical documentation
- Works effectively with adult and geriatric populations
- Is dependable, collaborative and patient-focused
