We’re hiring an experienced RN Case Manager for a hospital-based role in Glendale, Arizona. This position focuses heavily on discharge planning and coordinating post-acute care to ensure safe, efficient patient transitions.
Position Overview
Conduct initial case management assessments
Develop and coordinate discharge (DC) plans with patients, families, and care teams
Arrange post-acute services based on patient preferences and needs
Collaborate with physicians, nurses, and interdisciplinary teams
Ensure compliance with hospital policies and regulatory standards
Maintain accurate and timely documentation
Requirements
RN License – State of Arizona OR Licensed Master’s Level Social Worker in AZ
Minimum 5 years Case Management experience in an acute care hospital
Strong experience with discharge planning and post-acute placement
Excellent communication, coordination, and problem-solving skills
