Responsibilities
- Assess patients to determine occupational therapy needs and set measurable goals.
- Develop and implement individualized OT treatment plans with progress tracking.
- Document patient progress and outcomes in compliant electronic health records.
- Ensure compliance with regulatory and ethical standards in all activities.
- Collaborate with multidisciplinary team to coordinate patient care and referrals.
- Deliver evidence-based interventions to improve ADLs, mobility, and functional skills.
- Identify opportunities for professional development and pursue certifications.
- Utilize clinical reasoning to justify therapy plans aligned with business targets.
- Demonstrate strong on-site presence with proactive patient engagement and pipeline visibility.
- Monitor and report on patient milestones to support performance-driven culture.
- Educate patients and families on home-based programs and safety strategies.
- Participate in weekly case reviews to share learnings and best practices.
- Registered Occupational Therapist license required or eligible for state licensure.
- Experience developing and implementing OT treatment plans with measurable goals.
- Proficiency documenting progress in compliant electronic health records.
- Knowledge of regulatory and ethical standards; maintain compliance.
- Demonstrated ability to use clinical reasoning to justify plans aligned with business targets.
- Strong on-site engagement with patients and pipeline visibility for referrals.
- Experience in ADLs, mobility and functional skills rehabilitation.
- Proven collaboration with multidisciplinary teams for coordinated care.
- Willingness to pursue certifications and ongoing professional development.
- Excellent communication to educate patients and families on home programs.
- On-site role based in Lodi, CA — work from our local office and collaborate face-to-face with the team.
