Responsibilities
- Provide compassionate personal care to patients as a CNA on-site.
- Monitor and report changes in patient condition to supervisor promptly.
- Accurately document patient observations and care activities in charts.
- Collaborate with nursing and care teams to coordinate patient care plans.
- Demonstrate a growth mindset by seeking learning opportunities and skill advancement.
- Meet and exceed performance targets within assigned territory or area.
- Assist with daily living activities to support patient independence.
- Maintain a safe, clean, and organized care environment.
- Utilize care pipelines and workflows to forecast daily care needs and priorities.
- Provide empathetic, respectful communication with patients and families.
- Support on-site team with tasks to maximize efficiency and patient throughput.
- Maintain professional appearance and comply with facility policies and procedures.
- Certified Nursing Assistant license valid in the state of employment.
- Provide compassionate personal care and daily living assistance to patients onsite.
- Accurately document observations and care activities in patient charts.
