• Type: Skilled Nursing Facility (SNF 60 beds) / Long-Term Care (LTC 35–50 beds)
• Census: Typically 80–95 residents
• Semi-private rooms: ~20 rooms are non-private

• PointClickCare (PCC)

• RN: LTC 1:25 | SNF 1:13
• LPN: LTC 1:25 | SNF 1:13
• CNA: 1:10–15 (target ~1:10)

• BLS (Basic Life Support)

• Feeding tubes
• IV antibiotics (no titratable drips)
• Post-surgical patients
• Rehab patients
• Long-term care residents

• IV therapy (antibiotics/fluids)
• Patient assessments
• Medication administration
• Dressing changes & wound care (including wound vacs)
• Admissions and discharges (RN responsibility)
• Participation in care conferences encouraged (grand rounds/meetings)

• Vitals
• Activities of Daily Living (ADLs)
• No blood glucose checks

• Respiratory Therapist (RT) onsite Monday–Friday (day shift)
• PT/OT/SLP services available
• Case managers, social workers, dietician, MDS nurses, wound care nurse
• Dedicated pharmacy (24/7 availability)
• Omnicell med carts for medication management

• Hospitalist coverage after hours
• Nurse Practitioner onsite weekdays (M–F)
• On-call provider available 24/7
• Some private physicians follow residents

• RN/LPN shifts: 12-hour shifts (5a–5p / 5p–5a implied)
• CNA shifts: 8 or 12 hours
• Block scheduling may be accommodated
• Management schedules staff

• Every other weekend
• Every other holiday

• No call or standby
• Possible floating between units (preferred to stay on same unit during stretches)

• One 12-hour orientation shift
• Additional support available if needed after orientation

• No separate COVID unit (intermixed)
• PPE: N95 and PAPR available

• Any professional scrub color permitted
