Shift Hours: 9am-5pm
Hourly rate: $18 on W2 $46 (Travel Reimbursement) = $64.00/hrs
Travel rate: $675 $1,725.01 (travel reimbursement) = $2400.00
Job Description:
- Screens and assesses patients in an accurate and timely manner based on levels of care; determines patients' nutritional needs and status and provides follow-up care.
- Interviews patients and/or their families to obtain history of food habits and preferences.
- Interprets pertinent laboratory, physical and social data in the medical record, as it applies to nutrition care.
- Provides nutrition education and records patient's diet and diet instructions and the patient's understanding of the diet in the education section.
Experience:
- Minimal one year of experience.
Skillsets:
- Electronic medical charting systems – Point Click Care, Sigma Care etc.;
- MDS 3.0 Competent with electronic menu systems: CBORD, Net Menu, Geri Menu etc
Education:
- Bachelor’s degree in Dietetics, Nutrition, Foods and Nutrition, or a related field from an accredited college or university.
Additional Qualifications:
Certifications: Must hold a current Registered Dietitian (RD) credential through the Commission on Dietetic Registration (CDR), subject to Primary Source Verification.
