What are the responsibilities and job description for the MSW (Masters of Social Work) position at TALENT Software Services?
Duties
- Manage a caseload of 20-23 patients, ensuring effective care coordination and discharge planning.
- Conduct initial assessments within 24 hours of patient admission.
- Coordinate all post-acute care needs, working closely with medical teams and insurance case managers.
- Participate in care conference activities and collaborate with internal and external team members.
- Document pertinent information to facilitate care coordination and discharge planning.
- Assist patients in navigating the Department of Children and Families (DCF) process.
- Ensure quality care is delivered in a cost-effective manner.
- Required: Epic charting system proficiency.
- Preferred: Experience in case management/utilization review, adoption/foster care, and knowledge of major disease processes (e.g., arthritis, cancer, CHF, COPD, dementia, diabetes, HIV/AIDS, immune deficiency, neurological disorders, paralysis, psychiatric disorders, stroke).
- Ability to conduct biopsychosocial assessments and develop care plans.
- Strong documentation and communication skills.
- Knowledge of community programs and protective services.
- Minimum of 2 years of experience in a related field.
- 1 full year of acute care experience required.
- Preferred experience in Trauma Level I and II settings.
- Travel experience required; first-time travelers will not be considered.
Salary : $50 - $55
