About You
- Be able to do the job as described
- Experienced Nurse Practitioner seeing patients in office or home setting (no new grads)
- Some experience with hospice or palliative care is a plus
- Active Nurse Practitioner license in North Carolina (or ability to obtain)
- Strong ability to work independently in a home care setting
What You’ll Be Doing
- Provide in-home hospice and palliative care to patients
- Conduct patient assessments and develop care plans focused on comfort
- Support and educate patients and families on end-of-life care and advanced directives
- Collaborate with physicians, consultants, and interdisciplinary care teams
- Ensure delivery of high-quality, compassionate hospice services
