Position Summary

The Wound Care Nurse Practitioner is responsible for providing specialized assessment, treatment, and ongoing management of patients with acute and chronic wounds. This provider will collaborate with physicians, nurses, facility staff, and other healthcare professionals to develop individualized care plans and drive optimal healing outcomes.

The ideal candidate brings strong clinical judgment, comfort working independently in the field, and a patient-centered approach to care.


Responsibilities

  • Perform comprehensive wound assessments, including wound dimensions, characteristics, tissue type, signs of infection, and pain levels
  • Develop and implement individualized wound care treatment plans based on patient condition, history, and clinical needs
  • Provide advanced wound care interventions, including wound cleansing, debridement, dressing application, and wound closure techniques as appropriate
  • Prescribe appropriate medications, including topical agents, antibiotics, and pain management medications, as needed
  • Order and interpret diagnostic testing, including wound cultures, laboratory studies, and imaging, to support treatment decisions and healing progress
  • Educate patients, families, caregivers, and facility staff on wound care techniques, prevention strategies, and treatment plan adherence
  • Monitor wound healing progress, document findings thoroughly, and adjust treatment plans as needed
  • Collaborate with physicians, surgeons, vascular specialists, therapists, and other care providers to ensure coordinated treatment for complex wound cases
  • Maintain timely, accurate, and complete documentation in the electronic medical record
  • Stay current on wound care best practices, technologies, and evidence-based treatment approaches
  • Participate in quality improvement initiatives, audits, and other wound care-related clinical efforts


Qualifications

  • 2 years Nurse Practitioner experience
  • Active and unrestricted Nurse Practitioner license in the state of practice
  • Board certification as a Nurse Practitioner is required
  • Current DEA license and prescriptive authority, or ability to obtain upon hire
  • Wound care experience strongly preferred
  • Experience in post-acute care, skilled nursing, long-term care, rehab, or related settings preferred
  • Strong clinical assessment, diagnostic, and treatment planning skills
  • Experience performing wound care procedures such as debridement, dressing selection, and ongoing wound management preferred
  • Ability to work independently while managing a mobile, field-based schedule
  • Strong communication and relationship-building skills with patients, families, and facility partners
  • Comfortable using EMR systems, tablets, and mobile technology in daily practice
  • Valid driver’s license, reliable transportation, and ability to travel locally between assigned facilities
  • Organized, dependable, and adaptable, with a high level of professionalism and accountability
  • Strong personality and cultural fit for a collaborative, fast-growing organization

Salary : $85,000 - $120,000
