Qualifications:
Position Summary
Responsible for the coordination of administrative and discharge planning for all Talbott Recovery patients
Essential Duties and Responsibilities
- Establishes and maintains referral relationships.
- Contacting referral sources upon admission and confirms patient's needs/expectations regarding updates/information.
- Coordinates all communication and information exchange with referral.source.
- Develops a comprehensive and patient-focused continuing care plan for assigned patients.
- Timely and accurate input of contact information into established system.
- Ability to access, assess and report on requested and/or needed information.
Education and Experience
- Experience in alcohol/substance abuse/12-Step program.
- Experience in Discharge Planning, Case Management or Continuing Care Management
- Must have a High School Diploma or Equivelency; Bachelor Degree strongly preferred
