Responsibilities
- Provide routine and acute primary care in scheduled, same-day, and walk-in settings, ensuring patient-centered approaches.
- Maintain professionalism and uphold ethical standards, demonstrating compassion and adherence to agency dress and conduct policies.
- Utilize electronic medical records proficiently for documentation, order management, and clinical notes, ensuring timely and accurate record-keeping.
- Initiate and manage treatment plans for chronic illnesses such as HIV, diabetes, hypertension, and lipid disorders.
- Generate referrals to internal specialists and external services as needed, ensuring seamless coordination of care.
- Prescribe medications electronically in compliance with agency policies and state regulations, including administering medications per protocols.
- Perform diagnostic and therapeutic procedures according to accepted standards, interpreting lab results and follow-up on consultations.
- Educate and counsel patients on health management, preventive care, and medication adherence.
- Document clinical activities thoroughly within 48 hours of patient encounters.
- Participate in multidisciplinary team meetings, case conferences, and quality improvement initiatives.
- Provide clinical supervision to Advanced Practice Clinicians (Physician Assistants and Nurse Practitioners).
- Offer on-call coverage on a rotating basis and engage in community outreach, advocacy, and professional development activities.
- Medical Doctor degree and eligibility to practice in New York State.
- Board Certification or Board Eligibility in Internal Medicine or Family Medicine
- Achieve and maintain HIV specialist status (preferred), including completing at least 25 CME credits annually with 10 specifically related to HIV.
- Obtain a DATA Waiver within the first 90 days of employment for Buprenorphine prescribing authority.
- Demonstrated experience treating hard-to-reach populations such as homeless individuals, immigrants, substance users, and those with mental health conditions.
- Bilingual skills in English/Spanish or English/French are preferred.
