We are seeking a dedicated and compassionate full-time Social Worker (MSW) to join our home health team in Farmington Hills, Michigan.
This is an opportunity to make a meaningful impact by helping patients and families navigate emotional, social, financial, and environmental challenges while supporting their overall health and well-being.
What You Will Do
- Assess patients to identify psychosocial, emotional, financial, environmental, and community resource needs
- Develop individualized care plans based on patient assessments
- Provide patient and family counseling, support, and crisis intervention
- Assist with community resource coordination and referrals
- Support patients and families with advance directives and long-term care planning resources
- Collaborate with physicians, nurses, therapists, and interdisciplinary team members to ensure coordinated care
- Participate in case conferences and care planning discussions
- Complete timely, accurate documentation in compliance with agency, state, federal, CHAP, and Joint Commission standards

Qualifications
- Master’s Degree in Social Work from an accredited program
- Current Michigan Social Work license (LMSW or LLMSW preferred)
- Minimum 1 year of healthcare or home health experience preferred
- Strong assessment and care coordination skills
- Excellent communication and interpersonal skills
- Ability to work independently and exercise sound clinical judgment
- Valid driver’s license, current auto insurance, and reliable transportation
