Position Summary
The Intake Coordinator is responsible for managing the referral and admission process for home health patients. This role serves as the primary point of contact for referral sources, patients, families, physicians, and insurance providers. The Intake Coordinator ensures that referrals are processed accurately and efficiently while maintaining compliance with Medicaid, and other insurances.
Key Responsibilities
- Receive, review, and process incoming home health referrals from hospitals, physicians, case managers, and other referral sources.
- Verify patient demographics, insurance coverage, and eligibility for services.
- Coordinate with physicians to obtain required orders and documentation.
- Schedule patient admissions and communicate referral information to clinical staff.
- Maintain accurate patient records in the electronic medical record (EMR) system.
- Ensure all intake documentation complies with agency policies and regulatory requirements.
- Communicate with patients and families regarding the admission process and service expectations.
- Coordinate with nursing, therapy, and administrative staff to ensure timely start of care.
- Monitor referral status and follow up on pending documentation or authorizations.
- Build and maintain positive relationships with referral sources and healthcare providers.
- Assist with insurance authorizations and payer requirements as needed.
- Maintain confidentiality of patient information in accordance with HIPAA regulations.
- Perform other administrative duties as assigned.
- Train others as needed
- Tracks patients hospital transfers and obtains all required discharge paperwork
Qualifications
- High school diploma or GED required; Associate's degree in healthcare administration or a related field preferred.
- Minimum of 1–2 years of experience in home health, healthcare intake, medical office, or customer service.
- Knowledge of home health regulations preferred.
- Experience using EMR/EHR systems and Microsoft Office applications.
- Excellent verbal and written communication skills.
- Strong organizational and multitasking abilities.
- Ability to work independently and in a fast-paced environment.
