Role Description This full-time, on-site Physical Therapist role is based in Ashtabula, OH. The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide hands-on therapeutic interventions to improve mobility, function, and quality of life. Daily responsibilities include performing comprehensive assessments, documenting progress, updating care plans, and communicating regularly with patients, families, and other healthcare professionals. The role involves educating patients on injury prevention, home exercise programs, and self-management strategies, as well as coordinating care across the continuum, including possible home care recommendations. The Physical Therapist will also contribute to clinic operations, maintain accurate records, and uphold safety and quality standards.
Qualifications
- Strong foundation in Medicine and Physical Therapy, with the ability to apply evidence-based practices in a clinical setting.
- Experience in Rehabilitation and Patient Care, including assessment, treatment planning, and progress monitoring.
- Understanding of Home Care and continuity of care, including educating patients on home exercises and post-discharge plans.
- Current state licensure (or eligibility) as a Physical Therapist in Ohio and graduation from an accredited Physical Therapy program (DPT or equivalent).
- Effective communication and interpersonal skills to collaborate with patients, families, and multidisciplinary teams.
- Ability to manage a diverse caseload, prioritize tasks, and maintain accurate, timely clinical documentation.
- CPR/BLS certification and familiarity with electronic health record (EHR) systems are preferred.
- Previous outpatient, sports, or orthopedic physical therapy experience is beneficial but not required.
