SYNERGY HomeCare Delray Beach is a CHAP-accredited non-medical Home Health Agency serving clients in southern and central Palm Beach county. We have been in business for 16 years. We are currently seeking a per-diem part-time Supervisory RN to join our team.
As a Supervisory Registered Nurse with SYNERGY HomeCare, you will:
Conduct comprehensive home health patient assessment during Client Start of Care Admission and assessments on an ongoing basis in the clients’ homes. “Admission” includes completion of an agreement with the patient or the patient's legal representative to provide home health services. Implement plans of care to achieve positive outcomes.
Assist in handling Client emergencies, as well as client grievances and complaints.
Comply with applicable law and regulation.
Be part of the Professional/advisory group/Governing Body membership that involves annual review of policies and procedures and Biannual review of quality monitoring.
Participate in the ongoing accreditation-related duties.
Make supervisory visits in the clients' homes for purposes of overseeing caregivers as well as overseeing client care.
Perform Competency Evaluations of caregivers during initial hire of each caregiver and every year thereafter.
Document client care plans, changes in client conditions, results of quality visits, as well as keeping all client records up to date at all times.
Meet as necessary with staff to improve delivery of service.
Teach patients, family members and other caregivers as required.
Requirements:
An active RN license from the state of Florida.
Some experience in Home Care or Home Health care Assessments/Plans of care.
Clear Level 2 Background from state of Florida
Please note that this is a part-time position with the ability to schedule assessments and supervisory visits at your times of convenience. This may be a great position for an RN looking for part-time hours and supplemental income.
