Responsibilities:
- Provide personalized care to patients in their homes, including assistance with daily living activities, medication management, and mobility support
- Develop and maintain positive relationships with patients, families, and healthcare teams
- Monitor patient progress, identifying and reporting changes or concerns to the healthcare team
- Participate in care planning and collaboration with other healthcare professionals
- Maintain accurate and confidential records of patient care and communications
- Adhere to HIPAA guidelines and maintain patient confidentiality
- Perform other related duties as assigned
Requirements:
- Current certification as a Home Health Aide (CHHA) in the State of New Jersey
- High school diploma or equivalent required
- Ability to lift 50 pounds and move about quickly to respond to patient needs
- Excellent communication and interpersonal skills
- Compassionate and patient-focused approach to care
