We believe our caregivers are the heart of our agency and are committed to supporting them every step of the way.Position Summary:The Care Coordinator plays a key role in caregiver staffing, support, and scheduling, while also assisting with daily office operations. This position requires excellent communication, strong organizational skills, and a passion for building relationships with caregivers. The Care Coordinator will also provide direct client care when needed, helping to ensure seamless service delivery.Key Responsibilities:Caregiver Management & SchedulingRecruit, interview, and onboard qualified caregivers to meet ongoing client needs.Manage and maintain caregiver schedules to ensure reliable and consistent care coverage.Match caregivers to clients based on skills, availability, and compatibility.Track caregiver attendance and performance and follow up on any client or caregiver concerns.Build and maintain strong relationships with caregivers to support engagement and retention.Office & Administrative SupportAnswer phones, respond to inquiries, and provide professional communication to clients, caregivers, and community partners.Assist with data entry, filing, and maintaining accurate caregiver and client records.Support the office team in preparing reports and organizing internal communications.Utilize CRM systems such as AxisCare, ClearCare, WellSky, or similar platforms to manage scheduling and documentation.Client CareProvide direct care to clients as needed, including assistance with activities of daily living (ADLs), personal care, companionship, and light housekeeping.Fill in for caregiver shifts due to call-outs or last-minute coverage needs.Ensure adherence to individualized care plans and report any changes in condition or care needs.Qualifications:Experience in home care, caregiving, or healthcare staffing.CNA certification or similar caregiving background.Office or administrative support experience.Computer proficient, with the ability to quickly learn and navigate scheduling/CRM systems (familiarity with AxisCare, ClearCare, or WellSky highly preferred).Demonstrated ability to remain calm and focused under pressure, while maintaining a positive and proactive approach to problem-solvingExcellent written and verbal communication skills, with the ability to convey information clearly and concisely to both internal and external stakeholders.Positive and proactive attitude, with a willingness to learn and contribute to a collaborative work environment.Post-High school credentialValid driver’s license and reliable transportation.Why Work With Us?Competitive pay Possible remote work on select days Be part of a compassionate, mission-driven team.Opportunity to make a meaningful impact in both caregiver and client lives.Dynamic role with variety in daily responsibilities.Supportive and collaborative work environment.Apply Today:If you are passionate about supporting caregivers, keeping operations running smoothly, and stepping in to care for those who need it most, we would love to meet you.Join Synergy HomeCare of DeSoto County and help us bring comfort, independence, and dignity to our client at a time.
