Synergy HomeCare of Newport is seeking a Registered Nurse (RN) with a passion for community-based care and a desire to mentor others to join our dynamic team. This is a hybrid field and office role ideal for a nurse who values both clinical impact and caregiver development.
As the Assessment RN, you will lead the client onboarding process by conducting initial home care assessments and developing person-centered care plans. In addition, you will play a vital role in caregiver success by leading new hire orientation, ongoing training, and in-service education to ensure the highest standards of care.
Key Responsibilities- Perform comprehensive in-home client assessments
- Develop individualized care plans in collaboration with the care team
- Train and onboard new CNAs, PCAs, and Homemakers
- Lead monthly and quarterly in-service sessions aligned with RI state guidelines.
- Support caregiver compliance through documentation audits and skill refreshers
- Current RN license in Rhode Island
- Minimum 2 years of nursing experience, home care preferred
- Strong interpersonal and teaching skills
- Familiarity with RI Department of Health regulations is a plus
- Reliable transportation and comfort with field visits
