SYNERGY HomeCare of Mountain Lakes is seeking a compassionate and experienced Registered Nurse DON to join our well-established, growing home care agency. We primarily serve private-pay clients throughout Morris County and portions of Sussex, and northern Somerset counties.

Primary Responsibilities
- Conduct initial client assessments and develop individualized plans of care.
- Complete supervisory visits, follow-up assessments, and monthly client contacts within all regulatory timeframes.
- Provide ongoing supervision, coaching, and support to Certified Home Health Aides (CHHAs).
- Manage and supervise per-diem RN
- Perform Home Health Aide competency evaluations, skills assessments, and annual performance reviews.
- Assist with caregiver orientation, skills training, and refresher education.
- Build strong relationships with clients and families while addressing concerns and resolving care-related issues.
- Ensure compliance with all New Jersey regulations and accreditation standards.
- Complete accurate and timely clinical documentation.
- Educate clients and families on health management, medication administration, and disease prevention.
- Assist with client intake and other administrative responsibilities as needed.
Qualifications
- Active New Jersey Registered Nurse (RN) license.
- Bachelor's Degree in Nursing (BSN) with at least 2 years of nursing experience, or Associate Degree in Nursing (AAS/ASN) with at least 4 years of nursing experience.
- Previous experience in home care, hospice, community health, or a related field preferred.
- Excellent assessment, communication, and documentation skills.
- Strong interpersonal skills and a genuine passion for working with seniors.
- Ability to work independently, prioritize responsibilities, and manage time effectively.
- Valid driver's license, reliable transportation, and willingness to travel primarily throughout Morris County, with occasional travel to portions of Sussex, Warren, and northern Somerset counties.

Pay: $37.00 - $47.00 per hour
Expected hours: 30.0 – 40.0 per week
