Synergy Care is looking for an Occupational Therapist Assistant to fill a full-time position at Winning Wheels a SNF in Prophetstown, IL. You must be a licensed therapist or a new graduate.
Key Responsibilities
Patient Care & Treatment
- Carry out treatment plans established by the Occupational Therapist, using therapeutic activities and exercises.
- Assist patients with activities of daily living (ADLs), including dressing, grooming, feeding, and mobility.
- Utilize adaptive equipment and assistive technology to enhance patient independence.
- Encourage patient motivation and provide support throughout the rehabilitative process.
Documentation
- Maintain accurate, timely, and complete treatment records in accordance with facility policies and regulatory requirements.
- Document patient progress, goals, and outcomes for review by the supervising Occupational Therapist.
Collaboration & Communication
- Communicate regularly with the supervising Occupational Therapist regarding patient status and modifications to treatment plans.
- Participate in interdisciplinary team meetings when appropriate.
- Educate patients, families, and caregivers about therapy techniques and home exercise programs.

In order to be considered for this position you must be a graduate of an accredited school for Occupational Therapy Assistants or will graduate from such an institution within the next six (6) months. You must hold a valid National Board for Certification of Occupational Therapy (NBCOT) certification, and you must currently hold or be eligible for a valid state license (where appropriate) as an Occupational Therapy Assistant.
