RN Patient Navigator

Job Description

• The Patient Navigator is a key member of who will provide a single and centralized point of patient access to assist clinicians and patients in managing specialty and diagnostic referrals.
• When necessary, the Patient Navigator will lead patients through the health care system from the initiation of the referral, to obtaining prior authorization, scheduling appointments and tracking and following up on results.
  
  

Required Qualifications

• Registered Nurse (RN) - active and unencumbered license.
• Demonstrated experience in a multi-specialty physician office or comparable clinical setting.
• Excellent customer service and communication skills with a patient-centered approach.
• Strong computer proficiency, including use of electronic health record systems.
• Proven ability to manage multiple priorities and coordinate care efficiently.
• Demonstrated competence in acute care patient management, organization, and clinical judgment.
• Commitment to collaborative practice and professional relationships within an interdisciplinary team.
