Job Summary
The Family Nurse Practitioner plays a crucial role in our health care team, collaborating within an interdisciplinary model that includes physicians, pharmacists, registered nurses, behavioral health providers, case managers, and substance use disorder counselors. This position involves assessing and planning care for homeless clients with diverse health conditions. Key responsibilities include conducting comprehensive physical assessments, ordering diagnostic tests and procedures, formulating treatment plans, and obtaining specialist consultations as needed, while ensuring proper documentation. The selected candidate will provide care in both office/clinic settings and onsite for recuperative care patients. Additionally, they will be an essential member of the street medicine team, delivering care to unsheltered homeless individuals.
Supervisory Responsibilities:
- Oversees RNs, LVN, and MA’s in everyday operational duties.
- Ensures that staff adhere to established protocols and make adjustments as necessary.
Essential Duties & Responsibilities:
- Deliver general medical care for children, adolescents, adults, and geriatric patients.
- Conduct clinical intakes and assessments for recuperative care and shelter clients.
- Address chronic and acute primary care complaints, assessing medical urgency or emergency situations.
- Establish and document accurate medical diagnoses and seek specialty consultations as needed. Ensure timely documentation in electronic medical records.
- Order relevant diagnostic laboratory and radiology tests based on medical diagnoses or presenting symptoms.
- Discuss unusual or unclear symptoms and results with consulting physicians promptly.
- Schedule patient appointments for telehealth or in-person visits as appropriate.
- Perform diagnostic and therapeutic procedures within the scope of training and practice agreements with the supervising physician.
- Provide alternating on-call coverage to triage urgent lab results and develop appropriate care plans.
- Prescribe medications as appropriate and stay informed on current therapies.
- Work effectively in an environment with limited resources, using diagnostic tests judiciously.
- Collaborate with team members to establish best practices for efficient work execution.
- Engage in medical practices under the direction and supervision of a licensed physician, adhering to state laws.
- May perform RN duties, including medication administration, as needed.
- Collaborate with the supervising physician, initiating collaboration when necessary and recognizing when physician oversight is required.
- Perform other duties as assigned.
Qualifications:
Education & Experience:
- Graduation from an accredited Nurse Practitioner program with a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) with a Family Nurse Practitioner focus is required.
- 3-5 years of experience as a registered nurse and/or nurse practitioner in a community health or public health setting is preferred.
- Familiarity with outpatient family practice care and commonly prescribed medications (including controlled substances) is required.
- Ability to prescribe medications to patients of all ages, including children, adolescents, and adults, is required.
- Comfortability evaluating new patients and developing diagnoses and treatment plans with minimal supervision is preferred.
- Must be willing to provide rotating primary care services and recuperative care services as needed.
- Experience working with underserved populations facing socioeconomic barriers to healthcare is preferred.
- Fluency in Spanish is preferred.
- Valid, unrestricted CA driver’s license.
- Certification in Basic Life Support (BLS) from the American Heart Association (AHA) or the American Red Cross preferred.
- Knowledge and proficiency in Google Workspace, including Gmail, Google Docs, Google Sheets, and Google Calendar, is preferred.
- Must be comfortable working in a recuperative care environment.
- Must successfully pass a background check.
