What are the responsibilities and job description for the Case Management position at Sycamore Run LLc?
Possess a Bachelor’s Degree from an accredited college or university with a preference for training in the fields of special education, psychology, social work or in mental health counseling. At least one year of experience in programs serving individuals who have a diagnosis of Intellectual/Developmental Disability. Must possess good oral and written communication skills as well as a basic understanding and knowledge of computer skills.
Responsibilities include but are not limited to:
The Case Manager is required to remain updated on the important changes and or new requirements with the I/DD Waiver program by attending regular provider meetings or reviewing information sent by Accentra following the meetings. The CM will also be responsible for informing the participants, parents and/or guardians of the I/DD program changes and updates. The CM is responsible for notifying, convening, coordinating and chairing the individual’s IDT meetings and following all Waiver regulation guidelines. The CM is responsible for documenting all Case Management services and to ensure that all required original documentation for the I/DD Waiver Program is maintained at the agency and is accessible for State and Federal review. The CM is required to visit the participant at his/her resident face-to-face on a monthly basis as well as conduct a face-to-face Day Habilitation visit bi-monthly at the Day Habilitation location.
