The Nurse Practitioner provides primary care to participants. Performs physical and periodic assessments of participants develop and implement appropriate plans of care. Provides participant and family education.
Primary Functions
- Performs initial physical assessment of prospective participants. Appropriately identifies and documents problems in medical records. Performs periodic assessments of participants. Updates the plan of care, problem list and data sheet with appropriate documentation.
- Evaluates participants' physical complaints and orders appropriate diagnostic and therapeutic procedures.
- Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention and

- At the direction of the Medical Director, review medical outcomes/statistical and utilization data, including review of

- Master of Science of Nursing Degree
- Graduate of accredited Nurse Practitioner Program.
- Certified Registered Nurse Practitioner (NP) in Missouri.
- Registered Nurse (RN) in Missouri.
- 3 Years' clinical nursing practice in an acute or primary care setting.
- Current driver's license, auto insurance, and dependable transportation.
- Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact.
- 2 years’ experience as a Registered Nurse with extensive experience with the elderly.
- 1 year experience as a Nurse Practitioner in a geriatric setting.
- 1 year working with the elderly population.
Certified Registered Nurse Practitioner.
