Job Description:
The Clinic Manager manages flow and the operations of the clinic. Coordinates services with primary care team and acts as a primary liaison. Triages and assesses participants as needed and is the gatekeeper for daily clinic scheduling and flow of department. Provides skilled nursing care to participants; implements provider orders; develops appropriate plans of care; provides education and training to participants, caregivers, families, and associates; and communicates participant's condition to caregivers, primary care, and the interdisciplinary team (IDT).
Primary Functions:
- Participates as a member of the interdisciplinary Maintains regular attendance and participates in all interdisciplinary team and care plan meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.
- Evaluates overall effectiveness of the clinic, implementing changes and quality improvement as Monitor clinic departmental expenditures and meet budgetary expectations.
- Take lead in effectively communicating with interdisciplinary team, community agencies, contracted providers at meetings and throughout the day as necessary.
- Coordinate participant care with outside contracted service providers - nursing homes, assisted living, lab, oxygen,etc.
- Participates in weekend and after hours on-call duties, following up appropriately and updating the interdisciplinary team with changes and occurrences.
- Educate participants, family and caregivers in care, medications, treatments, and all other aspects of health education as needed.
- Educate associates in care and treatment of Reconcile Medication that providers have order.
- Manage and review participants' records, charts, pertinent information, and Maintains up-to-date and appropriate documentation in the participant's electronic medical record.
- Participates in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.
- Participates in in-service training and staff meetings as required.

Supervisor Duties and Responsibilities:
- Hires, selects, directs, supervises, and evaluates the performance of direct Handles discipline and termination, if necessary, of associates in accordance with policy.
- Oversees the daily workflow of the department.
- Ensures that the job responsibilities, authorities, and accountability of all direct reports are defined and understood.
- Assists in providing in-service education for the PACE KC associates.

Qualifications and Experience:
- Associate degree in Nursing.
- Active Registered Nurse (RN) It can be RN/LPN.
- (2) year's nurse management experience in a hospital, nursing home, or community-based setting.
- Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant
- (1) year working with the elderly population.
- Current American Heart Association Basic Life Support (BLS) with Automated External Defibrillator (AED) certificate or the ability to obtain within two weeks of hire, before having direct participant contact.
