Home Health Physical Therapy Assistant - Richmond, Hercules, Berkeley
Job Summary
We are looking for a compassionate and motivated Home Health Physical Therapy Assistant to join our team. In this role, you will work alongside physical therapists to provide individualized care in patients’ homes, helping to improve mobility, enhance safety, reduce the risk of falls, and support overall quality of life.
Responsibilities
- Assist licensed physical therapists in implementing individualized treatment plans for patients in the home health setting.
- Administer therapeutic interventions indicated by the plan of care including functional mobility training, strength training, aerobic conditioning, balance training, and ROM.
- Support patient care activities such as mobility assistance, gait training, and pain management strategies.
- Educate patients and caregivers on HEPs, safety precautions, proper DME usage.
- Communicate and collaborate with the supervising Physical Therapist and interdisciplinary care team to carry out established plans of care for patients in the home health setting across the lifespan.
- Adhere to standard precautions and infection control policies in the home setting, including proper hand hygiene, use of personal protective equipment (PPE), and cleaning and disinfection of reusable equipment between patient visits to ensure patient and clinician safety.
- Monitor patient progress toward plan of care goals and document responses to treatments accurately.
Requirements
- Valid licensure as a Physical Therapy Assistant (PTA).
- Preferred experience in home health, outpatient, SNF, and/or acute care settings.
- Familiarity with care plans for orthopedics, geriatrics, neuro, post-surgical patients.
- Strong patient care skills with the ability to work independently in home health settings.
- Excellent communication skills for patient education and interdisciplinary collaboration.
- Ability to adapt treatment approaches based on individual patient needs and progress.

Payment: Pay per visit basis

Pay: $90.00 - $105.00 per hour
