Position Summary
We are seeking an experienced Regional Patient Care Liaison to support strategic referral relationships, census growth, and market development across our Skilled Nursing Facilities in the Kansas region.
This role combines direct relationship development with hospitals and referral partners along with regional mentorship and coaching of Patient Care Liaisons to strengthen business development performance across the market.
The Regional Patient Care Liaison will conduct frequent in-person outreach with key referral partners, support facilities in strengthening their hospital partnerships, and provide hands-on coaching through ride-a longs and field mentoring to develop high-performing liaison teams.
The ideal candidate is a relationship-driven healthcare business development professional with experience in post-acute care, strategic account management, and team mentorship.
Key Responsibilities
Regional Strategic Relationship Development
- Develop and maintain high-level relationships with hospitals, health systems, physicians, case managers, and community referral sources across the Kansas region
- Serve as a regional liaison supporting facilities in strengthening key referral partnerships
- Conduct face-to-face outreach and in-person meetings with strategic referral partners throughout the region
- Represent the organization at healthcare networking events, hospital meetings, and community engagement opportunities
Census Growth & Market Development
- Support facilities in driving skilled nursing census growth through strategic referral development
- Identify opportunities to expand market share and strengthen preferred provider relationships
- Analyze referral trends, hospital discharge patterns, and admission conversion opportunities
- Collaborate with facility leadership and admissions teams to implement market growth strategies
Regional Liaison Mentorship & Coaching
- Provide ongoing mentorship, coaching, and professional development for Patient Care Liaisons
- Conduct ride-a longs and field visits to support relationship development and provide real-time coaching
- Partner with regional leadership to strengthen business development strategy and performance consistency
- Support accountability through review of outreach activity, referral development, and conversion outcomes
Admissions & Clinical Collaboration
- Work closely with Clinical Leadership, and Facility Administrators
- Facilitate communication between hospital discharge planners and facility teams
- Promote awareness of clinical capabilities, specialty programs, and quality outcomes
Market Intelligence & Reporting
- Provide insights to regional leadership regarding growth opportunities and challenges
- Utilize CRM systems to track outreach activities, referral sources, and strategic accounts
Qualifications
Required
- 3 years of healthcare business development experience
- Experience in Skilled Nursing, Post-Acute Care, or Healthcare Services
- Demonstrated success in new business development and relationship management
- Experience supporting strategic referral accounts and hospital partnerships
- Ability to mentor, coach, and develop business development staff
- Strong communication, relationship-building, and presentation skills
- Ability to travel frequently throughout the Kansas region
- Valid driver’s license
Preferred
- Experience working directly with hospital discharge planners and case management teams
- Knowledge of Medicare, managed care, and post-acute referral processes
- Experience conducting ride-a longs or field coaching with liaison teams
- Familiarity with CRM and referral management systems
