Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patient conditions, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) systems
- Administer medications and treatments as prescribed, ensuring proper documentation in compliance with home healthcare regulations
- Support patients with medical procedures such as catheter care, vital signs measurement, and basic wound care
- Aid with meal preparation, light housekeeping, laundry, and maintaining a clean, safe environment
- Provide companionship and emotional support to clients with dementia, Alzheimer's disease, or other cognitive impairments
- Follow patient transfer methods safely using equipment like Hoyer lifts or other assistive devices while adhering to patient safety protocols
- Maintain strict confidentiality of patient information in accordance with HIPAA regulations and uphold patient rights at all times
Skills
- Proven experience working with individuals with developmental disabilities, geriatrics, or in hospice care settings
- Strong knowledge of medical terminology, medication administration procedures, and vital signs monitoring
- Skilled in patient observation, documentation of medical records, and using EMR/EHR systems efficiently
- Ability to assist with complex care needs such as end-of-life support or dementia care with compassion and professionalism
- Familiarity with home health care assistance techniques including patient transfer methods and use of mobility aids like Hoyer lifts
- Excellent communication skills for effective client interaction and collaboration with healthcare teams
- Knowledge of home health regulations, safety standards including first aid, CPR, and basic life support (BLS) certifications preferred
- Capable of heavy lifting when necessary and maintaining a safe environment for both clients and staff
