**Description** Join a collaborative and mission‑driven Case Management Team (CMT) dedicated to supporting patients throughout their care journey. As a Case Management Associate, you’ll work closely with nurse case managers and social workers to help ensure patients receive the right care in the right setting. This role is ideal for someone who is organized, compassionate, and thrives in a fast‑paced healthcare environment where teamwork and communication make a meaningful impact. **Key Responsibilities:** Participate as an active member of the Case Management Team (CMT). Under the guidance of the nurse case manager and social worker, assist in facilitating appropriate patient placement. Utilize hospital and community resources to support continuity of patient care. Serve as a Utilization Management liaison. Collaborate effectively with Medical Staff, CMT members, the Business Office, Medical Records, and other hospital departments. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
