What are the responsibilities and job description for the Community Ombudsman position at SWAN?
Qualifications:
A bachelor's degree is preferred and experience in advocacy, aging, social services, health care or related fields. An associate's degree with the experience listed above will be considered.
Job Responsibilities:
The Community Ombudsman will investigate and resolve complaints involving residents of licensed long-term care facilities; participate in advocacy activities on behalf of residents; inform residents of their rights, provide information and education to residents, long-term care staff, and the community; network with other organizations and agencies regarding residents; rights, and complete all case notes in a timely manner in an electronic state operated documentation system.
Must be proficient in Microsoft Office and possess excellent word composition skills.Must have a valid driver's license, reliable transportation, proof of current automobile insurance, the ability to conduct visits in long-term care facilities, and be able to lift 25 pounds.
Employee is reimbursed at the current IRS rate monthly for travel incurred on-the job.
This is a full-time position - 32 hours per week with a four-day work week 8 am to 4 pm.
Employment is contingent on successful background checks through the Illinois State Police and the Department of Children & Family Services (DCFS).
