What are the responsibilities and job description for the Case Manager Interim position at SVS?
Social Vocational Services (SVS) has been serving adults with intellectual and developmental disabilities throughout California since 1977.
THE POSITION:
We're looking for full-time Case Manager at our Hollister office.
Eligible for growth incentives.
This position requires you to drive clients in SVS vehicles.
Supervise and monitor clients during daily activities (including, but not limited to: paid work, social skills building, art, recreation, volunteering)
Develop & monitor individual client plans within the Program.
Assist with personal hygiene needs as needed.
Assess, evaluate, implement and chart client's progress on goals and objectives.
MINIMUM JOB QUALIFICATIONS:
High school diploma, GED or equivalent
Good driving record and valid California driver's license
Post-job offer must obtain DOJ/FBI fingerprint clearance
Must pass post-job offer job-related physical exam including TB test
First Aid/CPR certification (SVS will provide)
Effective written and verbal communication
Pay: $22.00 per hour
