Salary: $17.13/hour

PRIMARY RESPONSIBILITIES:

Provide individualized and small-group support services to individuals with disabilities to promote community inclusion, skill development, volunteerism, employment readiness, and successful participation in community-based activities.

ESSENTIAL JOB FUNCTIONS:

  • Provide support services to participants in community, volunteer, employment, and recreational settings.
  • Build on participant's abilities and strengths based on: a) available past experience and information, and b) personal observations of the individual performing job tasks.
  • Provide on-site/off-site supervision of participants at all times unless otherwise indicated by the participant’s plan of service.
  • Gain a high-level knowledge of all elements of the assigned job(s), including general procedures and practices of the work site.
  • Implement participant service plans, goals, and support strategies as assigned
  • Complete required service documentation, progress notes, transportation records, incident reports, attendance records, and other agency documentation accurately and within established timelines.
  • Track and document participant time and attendance.
  • Ensure participant health, safety, dignity, and well-being while providing services in community and program settings.
  • Support participants in developing independent living, social, communication, and community participation skills.
  • Communicate participant progress, concerns, incidents, behavioral changes, and service needs to the appropriate supervisor in a timely manner.
  • Report incidents, accidents, participant injuries, and safety concerns in accordance with agency policy and established timelines.
  • Attend and successfully complete all required training and certification programs within established timeframes.

MARGINAL JOB FUNCTIONS:

  • May participate in plan review meetings concerning the participant.
  • May assist in monitoring program participants and other related work as required.
  • Other duties as assigned.

ADDITIONAL FOR DRIVERS:

  • Valid chauffeur's license and reliable transportation.
  • Observe and implement all driving, maintenance and safety factors related to the operations of the agency-owned vehicles.
  • Conduct preventative maintenance and safety checks before and after use on all agency-owned vehicles as assigned.
  • Safely assist participants entering, exiting, and riding in agency vehicles in accordance with agency training and procedures.
  • Drives gasoline or diesel-powered vans to transport rehabilitation program participants to and from community sites and volunteer sites.
  • Ensure that all appropriate documentation is completed in a timely manner. Maintains vehicle log according to agency standards.

QUALIFICATIONS:

  • High School Diploma, GED, or equivalence.
  • Experience in working with people with disabilities preferred.
  • Ability to work effectively with individuals with varying physical, intellectual, developmental, and behavioral support needs.
  • Ability to use basic computer systems, electronic documentation systems, email, and agency software.
  • Ability to be CPR and First Aid certified within 90 days of hire.
  • Possession of, or ability to obtain within 90 days of hire, a valid Michigan Chauffeur License and continued eligibility to operate agency vehicles in accordance with SVRC's driver qualification policy.
  • Successful completion of Criminal History Background Check and Drug Screen is required under the terms of agency employment.
  • Safe driving record.
  • Ability to safely transport participants and maintain eligibility to operate agency vehicles in accordance with SVRC policies.
  • Successful completion of agency sponsored defensive driving course.

PHYSICAL REQUIREMENTS:

  • Ability to walk, stand, and remain active for extended periods of time.
  • Ability to safely assist participants with mobility needs as permitted by training and agency policy.
  • Ability to lift and carry up to 25 pounds occasionally.
  • Ability to enter and exit agency vehicles repeatedly throughout the day.
  • Ability to travel throughout the community and work in a variety of community settings.

Candidates selected for an interview or other subsequent stage of the hiring process may be required to complete job-related assessments that measure technical proficiency and other competencies necessary for successful performance of the position.
