Under general supervision, the Team Leader at Coming Home Hospice oversees and makes assignments for the daily activities of Home Care Aides assigned to residential facilities and volunteers. The Team Leader is also responsible for providing quality patient care and the coordination of patient care activities at Coming Home Hospice.  Duties include, but are not limited to, the following:

  •  Updating and implementing individualized care plans for each patient
  • Assessing the patient and caregiver educational needs and provides them with education and information pertinent to the patient's diagnosis
  • Managing activities related to medications and performing skilled nursing activities and records care
  • Assigning, supervising, and coordinating Home Care Aide assignments during each shift


Qualifications:
  • Graduation from an NLN accredited vocational nursing proram required.
  • Current LVN or RN California license required.
  • Current CPR certification required.
  • Minimum of one year experience as a team leader in an acute care facility, skilled nursing facility, or other patient setting witha focus on Hospice, AIDS, or Oncology required.
