Overview
Under the supervision of the Program Director, the Community Case Manager/Educator provides short-term needs assessments, resource connections, and community education to individuals and families. This position delivers services through individual and group pop-up outreach events, connecting community members to essential resources, including food assistance, housing support, healthcare, and educational opportunities.
The role also plans and facilitates culturally responsive workshops and builds partnerships with local service providers to strengthen community access to supports.
The Community Case Manager/Educator maintains accurate and confidential records, supports data collection and reporting requirements, and contributes to continuous quality improvement.
Evening or weekend hours may be required to meet community outreach needs.

Pay: $55,000.00 - $58,000.00 per year
