Surry-Yadkin Residential Services is a small, non-profit organization located in Mount Airy, NC. We offer two group homes that support adults with intellectual and developmental disabilities, as well as a day program/gift shop. We provide services to a total of 12 clients.

The LPN is responsible for the medical treatment and care of the residents. Responsibilities include the following:
Assess resident’s continually for medical needs, medication side effects, toxicity, therapeutic response to drugs, as well as mental and well being.
Obtain physicians’ orders and place in the Medication Administration Record.
Nursing Notes on resident’s medical records updates.
Documentation of all health care performed by all and any health care givers such as primary care physicians, therapists, laboratories, hospitals, etc.
Coordinate all medical appointments and consultations and carry out all physician orders.
Attend meetings pertinent to the care and well-being of the clients including: Core Team, Human Rights Committee, Health and Safety.
Consult with the Pharmacy regarding all medications.
Ensure that residents receive on-going medical, dental and other related health care services following the guidelines standards and arrange transportation as necessary.
Review documentation and incident reports.
Requirements
Must be licensed to practice in the State of North Carolina as a licensed practical nurse.
Will have access to all health/clinical information for the individuals whom he/she serves.
These responsibilities are not all inclusive. The LPN shall be accessible 24 hours a day or will arrange for a relief nurse to be on-call for medical care.
Pay: $28.00 - $33.00 per hour
