The Community Resource Navigator, Delaware County (CRN) helps create the first impression of Surrey to people who make contact with Surrey Broomall and Surrey Media. The CRN represents Surrey’s motto of “neighbor helping neighbor” by interacting with others while presenting a friendly, respectful, caring, and professional manner. The CRN has sufficient knowledge of both Surrey programs and community resources to proficiently answer basic questions and/or refer the member to the appropriate staff person or other community resources.
Primary Duties
- Coordinate the Resource Navigation activities for Delaware County by working with both the Broomall and Media Surrey locations.
- Coordinate the Delaware County CHORE Connection Program. Oversee intake process, assess member eligibility and need, coordinate and organize volunteer supported workday projects, and maintain lists of the ongoing support needs of CHORE Connection members.
- Regularly (weekly) communicate with those identified as “at-risk” to reduce isolation. Try to determine what members might need and keep accurate records of calls and notes. Communicate members’ unique concerns to appropriate staff members.
- Schedule member appointments -- AARP, CVIM, PA Medi, and other appointments as necessary.
- Maintain ongoing “Fix-It” and “List Shopping” requests. Follow up with both members and volunteers.
- Take initiative to connect with local non-profits and service providers to build relationships and collect information about the services they provide and any eligibility requirements. Keep referral list(s) and resources up to date. The ongoing goal is to increase the number of vetted resources for our members and community needs.
- Work with Surrey Volunteer Coordinator to assist in recruiting, training, and supervising volunteers to support Delaware County program and member needs. Maintain open and ongoing communication with the Volunteer Coordinator.
Requirements:
- Must possess a strong sense of self direction and Initiative in their work.
- Must demonstrate excellent customer service skills and practices, including the ability to be discreet and diplomatic, as well as handle confidential information properly.
- Knowledge of Delaware County and other local or regional human service providers a plus!
- Must have a bachelor's degree or equivalent experience in a relevant field.
- Must have experience with volunteer management.
- Must have a valid driver's license and reliable transportation.
Pay: $50,000.00 per year
