Role Description This is a full-time, on-site Certified Nursing Assistant (CNA) role based in Providence, RI. The CNA will provide direct patient care in home and community settings, including assisting with bathing, grooming, dressing, toileting, and mobility. Responsibilities include measuring and recording vital signs, monitoring patient condition, and promptly reporting changes to licensed nursing staff. The CNA will support safe transfers, help with positioning, and assist with basic exercises as directed by the care team. Additional tasks include supporting meal preparation and feeding, maintaining a clean and safe environment, following care plans and infection-control protocols, and documenting care accurately and on time. The CNA will collaborate closely with nurses, patients, and families to promote comfort, independence, and respect.

Qualifications
- Current Certified Nursing Assistant (CNA) certification in Rhode Island and compliance with all state regulatory requirements.
- Demonstrated skills in direct patient care, including assistance with activities of daily living (ADLs), vital signs monitoring, and safe transfers.
- Strong communication, active listening, and interpersonal skills for working effectively with patients, families, and interdisciplinary team members.
- Ability to follow written care plans, document care clearly, and adhere to safety, infection-control, and confidentiality standards (including HIPAA).
- Reliable, organized, and able to work independently in patients’ homes while managing time and multiple tasks effectively.
- Physical ability to perform lifting, turning, and extended periods of standing and walking, with or without reasonable accommodation.
- Previous experience in home health, long-term care, or a related healthcare setting is preferred but not required.
- Valid driver’s license, access to reliable transportation, and willingness to travel within the Providence, RI area as needed.
