Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong understanding of social services, behavioral health, and crisis management to effectively assist clients in navigating their challenges and achieving their goals.
Responsibilities
- Conduct comprehensive assessments to determine client needs and develop personalized service plans.
- Provide ongoing support and case management services, including regular follow-ups to monitor progress.
- Facilitate access to community resources such as housing assistance (including Section 8), addiction counseling, and workforce development programs.
- Utilize motivational interviewing techniques to empower clients in making positive life changes.
- Offer crisis intervention and management services to clients facing immediate challenges.
- Collaborate with multidisciplinary teams, including social workers and mental health professionals, to coordinate care.
- Mentor clients by providing guidance in areas such as childcare and personal development.
- Maintain accurate documentation of client interactions, progress notes, and service plans.
Experience
- Previous experience in case management or a similar role is highly desirable.
- Familiarity with addiction counseling, behavioral health services, and crisis intervention strategies is beneficial.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent organizational skills and attention to detail for effective documentation and reporting.
- Ability to work independently as well as collaboratively within a team environment.
